Our home is designed to create a warm and welcoming environment, with comfortable living spaces and a range of amenities to promote wellbeing and independence. Our dedicated team of carers are passionate about what they do, and strive to create a homely atmosphere where residents feel valued, respected, and cared for. Care at Twelve Trees
At Meadowbrook, we offer both single and double en-suite bedrooms, thoughtfully designed to provide comfort and privacy. Every room comes with Wi-Fi, a digital TV point, and a telephone connection if needed.
We have modern assisted bathing facilities, complete with electric bath lifts and shower attachments, to help residents bathe safely and comfortably. Our discreet pager nurse call system replaces noisy buzzers and flashing lights, helping to create a calm, homely atmosphere.
Our dining rooms, private dining, lounges and hallways have all been recently refurbished, with cosy, easy-to-move furniture and a focus on comfort and sociability. Whether residents want to relax with a book, chat with friends, or join in with activities, they’re free to choose how they spend their day.
We also have hair salon and a treatment room facilities, during warmer months our air-conditioned lounge provide a cool, comfortable space to unwind.
For days out, our wheelchair-accessible car makes getting around easy and enjoyable, with a rear access ramp and a smooth, comfortable ride.

We’re also on hand to support residents with both planned and emergency medical appointments, offering reassurance and companionship whenever needed.
Promoting independence is at the heart of what we do. By truly understanding each person’s needs and preferences, we help residents enjoy an active, fulfilling lifestyle in a warm and supportive environment.
We also offer respite and short-term stays — whether it’s for recovery, a short break, or a trial visit, residents can expect a welcoming, safe space where they’re cared for with dignity and respect.

Mealtimes are an important part of daily life, and we take great pride in offering a variety of traditional, home-cooked meals prepared by our talented catering team. There’s always something to look forward to — including a daily selection of freshly made meals and delicious homemade cakes.
Residents can view the menu each day, and there’s always a choice available. We’re more than happy to accommodate individual dietary needs, lighter options, or specialist meals as part of our commitment to person-centred care.
We source our ingredients locally wherever possible, ensuring freshness and quality. Fresh fruit and vegetables are delivered in open boxes each morning, and meat and fish arrive several times a week to maintain the highest standards.
We also do our bit for the environment by recycling packaging and our cooking oil, it’s all part of creating a thoughtful, sustainable and enjoyable dining experience for everyone.

At Meadowbrook, we believe that meaningful activities are a vital part of daily life. Our dedicated activities team thoughtfully plans and delivers a wide variety of sessions each day, designed to engage, stimulate and bring joy to our residents.
From individual one-to-one activities — especially valuable for those living with dementia — to fun and sociable group sessions, there’s always something to look forward to. These activities help promote wellbeing, build friendships, and encourage physical and emotional health.
Our regular outings are always popular, offering residents the chance to explore local attractions, enjoy a coffee out, or simply take in the fresh air at a nearby park. Whether it’s crafts, gentle exercise, quizzes, pet therapy, or live music, we aim to provide something for everyone.

Meet the team

Ann Yates
Ann has worked in social care for close to 15 years have worked in care homes and home care for much of her career. Ann’s experience includes working with the elderly, in mental health and with learning disabilities at a variety of levels.
Ann began her journey with Twelve Trees in 2021 when she became the Deputy Manager of the Home Care business. She was quickly promoted and took on the role of Registered Manager, developing the company and the people to where they are today.
Ann transferred to Twelve Trees Meadowbrook in January 2025 and her knowledge and experience has been instrumental in developing the home and her team. Ann is currently studying for her NVQ Level 5 in Social Care, having already obtained her levels 2 and 3.
In her spare time, Ann is a keen golfer. She has also represented England in 8 World Championship Fishing competitions, winning gold in 2000.

Zoe Halpin
Zoe has worked for Meadowbrook since 2016, save for a short break in 2019 and was recently promoted to Deputy Manager in 2024.
Zoe has worked in Care for over 10 years and has a wealth of knowledge in the field. She is currently undertaking her Level 5 having already completed her Level 3.
In her spare time, Zoe enjoys camping, movies, documentaries and spending time with her family.
