A bit about us...

Twelve Trees is an award-winning, family-owned, luxury, 34 bed, all En-suite, Care Home in the leafy residential neighbourhood of Nether Edge in Sheffield. Whether you are using our Care Home or Day Care services, we provide the highest standard of care tailored to your individual needs. Our highly motivated and friendly team work together with residents and their families to ensure the care provided meets the needs and the goals of the people we look after. The home’s ethos is to provide personalised, exceptional individual care by an experienced and happy team, many of whom have been with Twelve Trees for over 20 years.


At Twelve Trees Care Home we offer both single and double en-suite bedrooms. All of our rooms are fitted with Wi-Fi, a digital TV point and a telephone, if required. We have facilities for assisted baths which is made easy with electric assisted units and shower attachments. In addition to this we have a modern pager nurse call system which removes the need for unpleasant lights and buzzers, making our environment more homely for our residents. We also have a wonderful conservatory which is nice to sit in and enjoy the warmth of the sun. If you go through the conservatory, it will take you outside where we have beautiful level gardens, with a variety of scented plants and herbs. Residents can take part in gardening and there are also quiet areas to sit and enjoy the outdoors with family and friends. Our dining rooms, hallways and lounges have recently been fully refurbished, and we have easy to move furniture specifically designed with comfort in mind. All of our communal areas have all been made to create a more comfortable and sociable environment allowing our residents to choose where they would like to go and how they would like to relax and enjoy their day. We have an in-house hair salon and treatment room which is ideal for visiting care professionals and our air-conditioned lounges are perfect for giving residents increased comfort during the hot summer months. For days out we offer transportation in our wheelchair accessible car, which is very comfortable, with large-opening sliding doors making it easily accessible for our residents.

Care Provided

Twelve Trees is an award-winning care provider, providing person centered care to all of our residents. We work closely with other healthcare professionals such as the Chiropodist, Dental/Optical professionals and Porterbrook Medical Centre who provide comprehensive healthcare services from within the care home. Our experienced care team support our residents by using a digital care platform, Person Centered Software, along with regular care plan reviews, bespoke risk assessments and falls prevention plans. As part of our continued commitment to our residents, we also support our residents to attend planned and emergency medical appointments, ensuring that they feel safe and comfortable at all times. Promoting independence is also vital at Twelve Trees and by delivering person centered care we believe that our residents are able to continue an active, enriched and happy life from within our home. Respite and short breaks are also available at Twelve Trees, whether convalescence care, a short break or even a trial visit, we offer a safe and welcoming environment for most respite needs.

Menus & Diets

All our traditional meals are home cooked by our amazing catering team. We offer a great choice of meals and homemade cakes every day. There is always a choice available, and our residents are able to view the menu each day outside the dining room. As part of our commitment to person centered care, lighter meals, dietary requirements and specialist meals can also be easily accommodated. All our ingredients are locally sourced for freshness and quality. All our vegetables and fruit are delivered in a large open box for use that day, with fresh meat and fish being delivered multiple times a week. We try to do our bit for the environment too by recycling boxes and plastic from food products, either by being collected by the recycling company or being used during the activities within the home.


At Twelve Trees we have our own dedicated activities team who plan, create and deliver activities on a daily basis. Our Activities Coordinators, Carole and Viv have a wealth of experience and knowledge, having worked for Twelve Trees for well over 30 years between them! Activities can be on a one-to-one basis, allowing the team to focus on the individual, which is particularly beneficial for residents with dementia. Group activities provide a chance for residents to socialise, create relationships and improve physical and emotional health. We arrange trips and outings for our residents on a regular basis including parks, shopping, coffee shops, local landmarks and other attractions. Often our activities include craft sessions, chair aerobics, pet therapy, games and quizzes, sing along and live entertainers. Nicola Smith

Registered Manager


Nicola has been part of the Twelve Trees family for over 25 years and is the Registered Manager at our Care Home.

Nicola has many years of experience and has worked in various positions in care and for the NHS before becoming the Registered Manager at Twelve Trees. Nicola has a has a Level 5 Diploma in Health and Social Care and has completed a number of supporting qualifications including, Leadership and Management and Specialist Care Training.

Nicola has a dedication to providing the highest quality standard of care for every one of our residents and this is bolstered by over three decades of experience in the Health and Social Care sector.

Maria Snape

Senior Administrator

Maria has worked for Twelve Trees for over 10 years and has a wealth of knowledge and experience within Business Administration.

Maria is the first point of contact within the home, providing first class customer service to all our employees, the residents and their families.

Cheryl Daly

Deputy Manager

Cheryl has worked for Twelve Trees for over 20 years, joining us in February 2002.

Cheryl has extensive knowledge and experience in her role as Deputy Manager with a Level 3 in Health and Social Care she is currently working towards her Level 5 and holds a number of other enhanced qualifications.
